A 20 year old girl reported to our hospital seeking cosmetic correction of her broad, flattened nose. Maxillofacial Surgeon Dr. S. M. Balaji successfully corrected her nose by closed augmentation rhinoplasty. The dorsum of nose was raised using costochondral autograft obtained from rib cartilage. Patient is very happy to have her nose corrected and made pristine without any scars on her face.
A 13 year old girl from Dubai was brought to our hospital with the complaint of defective shape of the nose due to birth defect. Her parents consulted many plastic surgeons for cosmetic correction of her nose, but they were not convinced. Finally she was brought to our hospital for correction of the nose defect. Maxillofacial Surgeon Dr. S. M. Balaji successfully corrected her nose defect by following Denonvilliers technique. The left alar rim of the nose was reconstructed by rotating the “Z” shaped flap from the superior portion of ala of the nose. Results are immediate and her nose had a pleasing attractive appearance. Patient and her parents are happy to have the entire nose correction without any scars on her face. A girl with unilateral cleft lip and palate previously operated elsewhere came to our hospital wanting to correct her deformed nose shape and remove the scar on her lip to enhance her appearance. Maxillofacial Surgeon Dr. S.M. Balaji successfully performed a combined surgery to correct the lip and nose defects. Using a transcolumellar approach, a columellar strut from a rib graft, was interpositioned between lateral nasal cartilage to straighten the columella, raise the nose height and reconstruct the collapsed left ala of the nose. Thus deviated nasal tip was brought to the midline of the face. Scar on the upper lip left side was also revised. Following the surgery, the girl’s facial profile was enhanced with the reshaped nose and corrected lip scar. Orthognathic surgery or corrective jaw surgery is done to reshape the jaws and achieve a more balanced alignment. This improves the teeth alignment & facial profile and enhances one’s appearance and confidence. Moreover this surgery is done from inside the mouth so there are absolute no scars on the face. When the jaws are realigned, the soft tissues in the chin, cheeks, lip and tip of the nose move accordingly. Immediate results are seen following this type of jaw correction. Also there is no relapse. Once the jaws are correctly positioned, the architecture of the facial skeleton is changed and a harmony between the facial features is achieved giving a more pleasing, attractive appearance. When the lower jaw is severely protruding and not in proper alignment with the upper jaw, it causes difficulty in biting, chewing, speaking and also affects the facial appearance. With corrective jaw surgery or orthognathic surgery, the jaws can be reshaped and aligned in normal balanced occlusion.
Bilateral sagittal split osteotomy (Obwegeser’s) is done for surgical correction of the skeletal defect of protruding lower jaw. In this the lower jaw is reshaped and set in precise alignment to enhance the appearance and achieve proper bite. This procedure is done from inside the mouth so there are no scars. Macroglossia is a condition in which the tongue is abnormally enlarged beyond the normal resting position. Causes include genetic factors or medical conditions like hormone disturbances, infections etc. The enlarged tongue causes difficulties in breathing, chewing, swallowing, speaking and also disturbs teeth position. Due to the protruding tongue, the mouth remains open causing repeated throat infections and drooling of saliva. Moreover the appearance also affects the psychological and social well-being of the child. If there is an underlying cause, that should be treated first. If the problem is genetic or syndromic, tongue reduction surgery or glossectomy is done.
Maxillofacial Surgeon Dr. S.M. Balaji performs tongue reduction surgeries ensuring preservation of taste sensation and tongue functions. After surgery, breathing, chewing and swallowing becomes easier, it becomes easier to close the mouth and facial appearance improves. When the upper jaw is disproportionately larger than the lower jaw, it affects the facial profile and gives an unattractive appearance. Also the upper teeth are excessively protruding and there is inability to close the mouth & bite normally. This can be surgically corrected by orthognathic surgery. Using specialized Le Fort I technique the upper jaw is made proportionate and more accurately aligned with the lower jaw. This is done from inside the mouth so there are no scars. The results are immediate and the patients get a renewed confidence with their enhanced appearance.
Cleft lip occurs when parts of the face forming the lip fail to fuse normally before birth. Incomplete cleft lip is when the gap in the lip does not extend up to the nose. Cleft lip correction is done usually when the baby is about 3 months old using modified Millard’s surgical technique. Maxillofacial Surgeon Dr. S.M. Balaji has successfully treated innumerable patients from all over the world born with cleft lip and cleft palate.
