Maxillofacial surgeon Dr. S.M. Balaji skillfully performed the cleft rhinoplasty. A costochondral cartilage graft was harvested and the dorsum of the nose was augmented using the graft. The prominent lower lateral nasal cartilage was removed on both sides to correct the ala defect and reconstruct the structure of the nostrils. The surgery was done from inside the nose so there was no scar. Immediately after surgery, the nose appeared sharp and defined and the boy was very happy with the scarless results.

A 12 year old boy with bilateral cleft lip and palate reported to our hospital with the complaints of defective upper lip and ill-defined nose tip with depressed nasal bridge. His parents were not happy with the boy’s appearance and he was not able to spell few words due to defective upper lip which made him more depressed. Maxillofacial Surgeon Dr. S. M. Balaji skillfully planed to correct his upper lip defect as well as his ill- defined tip of the nose by following Abbe flap technique. Surgery was done in two stages. In the first step, prolabium was raised to increase the columellar length and nasal tip projection. And the cleft lip defect was corrected with Abbe flap technique were lower lip tissue was raised and attached to the upper lip defect region. In the second step, achieving good healing after 25 days the upper and lower lip was surgically separated and thus nose as well as lip defect correction was done simultaneously without many surgical scars. The boy and his family was very happy to see the enhanced appearance of the nose and well aligned upper and lower lips without any surgical scars. A 30 year old man reported to our hospital wanting to correct his nose profile which was very broad and flat to his face. He also wanted to correct his nose tip which was ill-defined that gave him more aged appearance. Due to his defective nose profile he was very self conscious about his appearance. Maxillofacial Surgeon Dr. S. M. Balaji skillfully planned to correct his nose by following Augmentation Rhinoplasty technique with alar base correction. Costochondral graft was harvested from the ribs, shaped and used to reconstruct the nose to increase the height. Thus proper shape of the nose was achieved. Alar base was corrected with Weir excision and the nostrils were made defined and symmetrical. Entire nose correction was made within the nose (closed rhinoplasty technique); there are no scars on the face. Results are immediate and the patient was very happy to see his enhanced profile of the nose without any scars. This dramatically improved his appearance and self confidence. A 25 year old man reported to our hospital with the complaints of broad and flattened nose which gave him an aged appearance. He was not happy with his nose profile. Maxillofacial Surgeon Dr. S. M. Balaji skillfully planned to correct his nose following Augmentation Rhinoplasty technique with alar base correction. Rib cartilage graft has been harvested and shaped to correct dorsum of the nose. Alar base was corrected by Weir excision technique. Thus surgery was done within the nose (closed rhinoplasty technique); there are no scars on the face. Results are immediate and the patient was very happy to see his enhanced appearance of his nose without any scars. This dramatically improved his profile and renewed his confidence. A 25-year-old girl reported to our hospital seeking to correct her nose defect to enhance her appearance. The middle portion of her nose appeared collapsed and depressed giving the nose an unattractive, concave, “saddle-like” appearance. Maxillofacial Surgeon Dr. S.M. Balaji skillfully performed the rhinoplasty to correct the nose defect. A costochondral graft was harvested and used to reconstruct the collapsed dorsum of the nose. An intranasal approach was used to avoid scarring on the face. Immediately after surgery, the contour of the nose improved greatly. The nose was made sharp and pristine. Facial appearance improved distinctly and the girl was very happy to have a more attractive facial profile without any scars. A 27-year-old girl reported to our hospital seeking expert treatment for her flattened nose. She wanted to improve the appearance of her nose for a more attractive facial profile.
Her nose was board and flat with the middle portion of the nose appearing obscure. Maxillofacial Surgeon Dr. S.M. Balaji expertly performed the rhinoplasty surgery. A costochondral graft was harvested and used to reconstruct the dorsum of the nose. The surgery was approached from inside the nose so there was no scarring on the face. Immediately after surgery, the nose appeared sharp, pristine and overall made the face more attractive. The patient was very happy to enhance her looks without scars. A 22 year-old-girl born with unilateral cleft lip and palate reported to our hospital seeking expert treatment for her cleft nose defect. Due to the cleft, the left side of her nose was deformed which affected her appearance and she wanted to correct her facial profile.
Maxillofacial Surgeon Dr. S.M. Balaji skillfully performed the cleft rhinoplasty. The left ala of the nose was severely depressed. Costo-chondral cartilage was harvested and shaped to reconstruct the nasal framework. Through an intranasal approach the depressed left ala was lifted up by placing the shaped graft. The cartilaginous strut graft was positioned between the medial crura to reconstruct the base of the nose. Immediately after the surgery the shape and appearance of the nose improved greatly. Also since the surgery was done from inside the nose, there were no scars. The girl was very happy with her improved appearance & facial profile. A 25 year old man reported to our hospital wanting to correct his deviated abnormally large sized nose. Because of his appearance of the nose, he was very depressed and he wanted to make his nose pristine without any scars. Maxillofacial surgeon Dr. S. M. Balaji skillfully planned to correct the nose by following reduction rhinoplasty technique and alar base shaped by using weir excision. Bilateral incisions are made inside the nose and connected by transfixation intercision. Deviated nasal septum was exposed and dissected in such a way that the anterior part was excised in a triangular manner. Both lower lateral cartilages (cephalic portion- foot plate of medial crus) were excised and removed leaving the caudal portion intact. Both side upper nasal cartilages were divided in the midline through a cut made in the superior part of upper cartilage of the septum (medial osteotomy). Dorsum of the nasal bone was osteotomized by lateral osteotomy technique and rough surface of the nasal bone was smoothened with the rasp. Weir excision was made to shape the ala of the nose. And the flaps were approximated in layers. Results are immediate. The surgery was done within the nose resulting scar less appearance and the nose was shaped to normal size and shape which well blended to his facial appearance and also enhanced the profile of the face. A 28 year old male reported to our hospital with the complaint of deviated nose with depressed nasal bridge and left nostril. His nose was flattened with the ala in the varying level in both sides. Maxillofacial Surgeon Dr. S.M. Balaji corrected his nose by following Augmentation Rhinoplasty technique with Alar base correction. Cartilage graft has been harvested and sculpted for the correction of depressed nasal bridge. The nasal septum and foot plate of medial crus is removed surgically from the left side along with the correction of alar base. The surgery was done within the nose so there are no scars on the face. Results are immediate and he is happy to have a renewed confidence to have a pristine nose with a new, improved look. A 15 year old boy with unilateral cleft lip and palate from Tirupur reported to our hospital wanting to correct his nose and alignment of his teeth. Primary lip and palate repair was done in his hometown itself during his childhood. He and his parents were more upset regarding his scar results after lip repair which affect his moustache growth as well as his broad asymmetrical nose made his face look disproportionate. Maxillofacial Surgeon Dr. S. M. Balaji skillfully planned to correct his cleft defects to overcome the future growth huddles. Considering their level of distress they had in the results of previous surgeries, multiple cleft defects were planned to be corrected in a single surgery. Deviated nasal septum was aligned through intra oral approach using chondral graft obtained from his rib. Thus his nose defect was shaped to normal shape and size. His cleft alveolar defect was closed with miracle protein rhBMP-2. His lip defect which was due to incorrect approximation of mucosal flaps leaving the vermillion of the lip defective. Thus, vermillion defect was corrected and scar formed due to incorrect technique was removed precisely with very minimal scar formation. Results are immediate showing enhanced appearance of his face due to pristine nose correction and scar revision. |
