Successful Operculectomy & Unerupted tooth brought to its normal position through orthodontics18/3/2014 A 10 year-old boy was brought to our hospital by his parents with a complaint of an unerupted upper front tooth. The front tooth on the right side had erupted normally but the tooth on the left side had not appeared. A digital X-ray taken showed an over-retained deciduous tooth (milk tooth) preventing the normal eruption of the permanent maxillary left central incisor. The deciduous tooth was extracted and with the operculectomy procedure the thick overlying soft tissues were uncovered to expose the “hidden” permanent tooth. Using an orthodontic appliance, the tooth was brought down into normal position maintaining its vitality. This was performed at an earlier stage itself to prevent the impaction of central incisor. Results are immediate and the patient’s teeth color brightened considerably. A 33 year old female came to our hospital with a complaint of prolonged pain for past 6-7 years and mobility of her upper front teeth. Her root canal treatment done elsewhere had failed and she developed an abscess. Digital radiograph was taken that showed two radiolucent (indicating bone loss) areas close to the apices of upper front teeth on both the right & left side. Maxillofacial Surgeon & Implantologist Dr. S.M. Balaji planned a combined treatment. The upper right & left lateral incisors & canines were root canal treated with apicoectomy. The mobile upper central incisors were extracted. The cyst lesions were thoroughly enucleated. A bone graft was taken from the symphysis (anterior region of the lower jaw) was used to completely fill the bone defect. Simultaneously, two dental implants were placed in the sockets of the extracted teeth itself. She was given a temporary prosthesis. Following osseointegration of the implants with the jaw bone, permanent ceramic crown prosthesis will be given. Effective treatment of such complex cases requires expert care & skill.
