Cosmetic rhinoplasty for deviated nose without scars

22 year old men reported to our hospital seeking to correct his deviated nose which greatly affected his facial appearance. He gave a history of road traffic accident 2 years back where he had severe injury on the nasal bones. He was very much disappointed as his nose was completely deviated to the right side giving asymmetric facial appearance.
Maxillofacial Surgeon Dr. S.M. Balaji successfully performed the cosmetic rhinoplasty. Vertically nasal septal cartilage height was trimmed by medial osteotomy to reduce the height of the nose. Deviation along with the width of the nose was reduced by lateral osteotomy technique. Thus deviated nose and size of the framework of the nose was corrected from inside the nose to avoid any scars on the face (closed rhinoplasty technique).
A balance between the nose shape and facial profile was achieved for a more pleasing appearance. The patient was very happy to have a narrower, shapely nose that enhanced his facial profile with absolutely no scarring.

A 23 year old woman reported to our hospital seeking to correct her deviated nose which greatly affected her facial appearance. She was very much disappointed as her nose was completely deviated to the right side giving asymmetric facial appearance.

Maxillofacial Surgeon Dr. S.M. Balaji successfully performed the cosmetic rhinoplasty. Costochondral rib graft obtained to augment the dorsum of the nose. Vertically nasal septal cartilage height was trimmed by medial   osteotomy to reduce the height of the nose. Deviated nasal bone was corrected by lateral osteotomy technique. Thick facial scar adhered to underlying bone corrected for enhanced appearance

This is a case of huge dermal scar revision. Due to road traffic accident the patient had a skin abrasion 5 months back. He was very unhappy due to a depression in the lower jaw and upper neck region on the left side with absence of hair growth in the affected region, and a thick scar that was adherent to the underlying bone, which was affecting his appearance. He wished to have the complete scar revision to enhance his facial appearance.
Maxillofacial Surgeon Dr. S.M. Balaji successfully removed the thick scar tissue which was strongly adherent to the underlying bone. The depression was reconstructed by the mobilization of platysma and skin by following double layer closure. The patient was happy that his appearance was greatly improved.

A 26-year-old man came to our hospital seeking to correct the shape of his nose to enhance his appearance. His nose was very large and bulbous.

Maxillofacial Surgeon Dr. S.M. Balaji successfully performed the reduction rhinoplasty to correct the nose shape. Lateral osteotomy was done and lower lateral nasal septal cartilage was removed to reduce the bulky nose and decrease the size of the nose tip. Weir excision was done to narrow the wide nostrils. 25 year old man reported to our hospital seeking for cosmetic correction of his right ear defect. He consulted many plastic surgeons for the surgical correction but was not satisfied.

32 year old American women reported to our hospital seeking to correct the shape of her nose. Her nose was disproportionately large and which affected her appearance. She sought to enhance the appearance of her nose for a more pleasing look.

Maxillofacial surgeon Dr. S.M. Balaji successfully performed the reduction rhinoplasty using closed rhinoplasty technique to reduce the size of the nasal framework. Intranasally, bulky tip of the nose was reduced by removing the caudal portion of nasal septal cartilage and asymmetrical nostrils were corrected by removing the footplate of medial crus on the left side. Lateral and medial osteotomy was done to reduce the width of the nose. The surgery was done from inside the nose to avoid scarring. Immediately following surgery the nose appeared proportionate greatly enhancing the facial appearance.

A 25 year old male reported to our hospital with the complaint of depressed nasal bridge and broad base of the nose with bulky tip of the nose.

A 23-year-old girl came to our hospital seeking to correct her nose and chin defect. Her chin was very retruded and the tip of her nose was very bulky. She was very self-conscious about her appearance and wanted to improve her facial features.

Maxillofacial Surgeon Dr. S.M. Balaji successfully performed a combined nose and chin correction. Advancement genioplasty was done to correct the retruded chin. The chin bone was sectioned, brought forward and fixed in a more balanced position. A young man came to our hospital seeking corrective jaw surgery to enhance his appearance. His lower jaw was excessively protruding causing an incorrect teeth occlusion that caused difficulties in biting and chewing. Additionally his chin bone was deviated away from the midline giving his face an asymmetric, crooked appearance.
Maxillofacial surgeon Dr. S.M. Balaji planned to correct the jaw defect with a combined bilateral Obwegeser’s sagittal split osteotomy and genioplasty. The mandible was set back to correct the alignment and bite using bilateral Obwegeser’s sagittal split osteotomy. With genioplasty, the deviated chin bone was corrected. Surgical facial enhancement for forehead depression and deep nasal bridge

A 32 year old man from Bangladesh reported to our hospital for his forehead depression correction. Due to a road traffic accident few months back he had frontal bone fracture and posterior displacement of the bone. He wanted to correct the forehead defect.

After thorough clinical and radiological assessment, Maxillofacial Surgeon Dr S M Balaji diagnosed that due to fracture of the outer table of the forehead (frontal) bone he had a depression and there was a deep nick in the nasal bridge also which required surgical correction. Surgery was done through the previous scar in the forehead and costochondral rib graft was used to augment the defect. Bridge of the nose was also enhanced using rib graft through the same incision.  Patient feels very happy to have both forehead and nose correction done in the same surgery improving his appearance without much surgical marks on the face.

A 22-year-old girl from Bihar was brought to our hospital by her parents for expert correction of her cleft nose & lip defect to enhance her appearance. Her nose was flattened and collapsed on the left side and there was an unsightly scar on the lip which affected her appearance. She was previously operated elsewhere for her unilateral cleft lip and palate in her childhood.

Complete jaw reconstruction of missing left side Mandibular ramus, condyle and temporomandibular joint to correct face asymmetry

A 13-year-old girl was brought to our hospital by her parents for specialized jaw reconstruction surgery. The girl’s lower jaw was deviated to the left side causing asymmetry of the face. Since birth the back portion of the left side lower jaw (Mandibular ramus and condyle) was missing causing difficulty in speaking and chewing. Also the lower jaw deviated to the left side while opening the mouth. The girl’s appearance was also affected making her self-conscious.

The 3D CT scan revealed complete absence of the posterior portion of the lower jaw left side. The left side jaw joint or temporomandibular joint was also absent. Maxillofacial Surgeon Dr. S.M. Balaji successfully performed the jaw reconstruction surgery. A costochondral rib graft was harvested and used to reconstruct the missing left mandibular ramus, condyle and temporomandibular joint. Immediately following jaw reconstruction, the face appeared symmetrical improving her appearance. The girl will require post-surgery physiotherapy to rehabilitate the jaw joint function.

Combined nose and jaw correction for enhanced appearance

A young man came to our hospital wanting to correct the shape of his broad, bulky nose that gave his face an unattractive appearance. Also his lower jaw was severely protruding making it difficult for him to bite, chew and speak well. He wanted to correct his jaw defect also.

Maxillofacial Surgeon Dr. S.M. Balaji performed a combined nose correction and jaw correction surgery. The large bulky nose was reduced and made more proportionate and sharp with reduction rhinoplasty. Using Obwegeser’s sagittal split osteotomy the protruding lower jaw was set back and into proper alignment with the upper jaw thereby correcting his bite. Nose and jaw correction was done from inside the nose and mouth respectively so there were no scars.  The man was very happy that his appearance improved greatly and that both his nose and jaw defect were corrected simultaneously.

Successful cosmetic cleft nose correction with lip revision

A 28-year-old man from Kolkata reported to our hospital seeking specialized treatment to correct his cleft nose defect. He was previously operated for cleft lip and palate elsewhere in his childhood. Due to the cleft defect his nose was collapsed on the right side and it affected his facial appearance. He wanted to have unsightly lip scar correction also in the same surgery.

Maxillofacial Surgeon Dr. S.M. Balaji successfully performed the rhinoplasty or nose correction. A costochondral graft (rib graft) was harvested and used to reconstruct the collapsed nasal framework. The surgery was approached from inside the nose so there will be no scar formation. Alar base on the right side was at a lower level compared to other side due to cleft defect. This was corrected by raising the floor of nose intraorally using costochondral graft thus nose was made symmetrical on either side. 

Huge unsightly lip scar defect which was formed due to previous lip surgery done elsewhere also corrected and shape of the vermillion was reconstructed. Immediately after surgery, the nose appeared sharp and definite giving a more pleasing appearance. The patient was very happy to have his nose and lip defect corrected without much visible surgical marks.

A 22 year old man reported to our hospital seeking cosmetic correction of his left eyebrow. One year back, due to an accident he had a cut injury in the left eyebrow for which he had a first aid at some hospital elsewhere. The cut ends of the eyebrow were misaligned which made him very conscious about his facial appearance. And also he wanted to correct the scar which was wide and unsightly formed due to previous surgery.

A 25 year old man from Bangladesh reported to our hospital seeking cosmetic correction of his forehead bossing. He had this prominence from his childhood itself. He was very upset of his profile view where the forehead prominence was very obvious. He wanted to reduce this cosmetically without any scars.

After thorough clinical and radiological examination, Maxillofacial Surgeon Dr. S. M. Balaji diagnosed that due to frontal sinus enlargement the frontal bone was bulged out. Dr. Balaji corrected this defect by raising bicoronal flap and the outer table of frontal bone prominence was reduced. Cosmetic nose correction for broad flat nose without any scars

Surgical correction of nose deformities also known as the “Nose Job” requires expertise and experience.  The surgeons estimate the type of defect - either excess or deficit of the height, deviations, depressions and abnormally angled noses. The patient’s expectation of his/her nose is understood after careful examination.

Shown is a case with a deficiency of the bridge portion of nose. Note the midlevel of the nasal bone, where there is a deficiency creating a huge impact on the patient’s facial profile. This gives the nose a saddle-shape. Tip of the nose appears bulky and everted in the profile view; base of the nose appears broad. Maxillofacial Surgeon Dr. S.M. Balaji planned a closed rhinoplasty technique wherein the surgical approach is through the nose to prevent unsightly scars.  

Appropriate measurement of nose was made and the amount of augmentation needed was noted. Autologous graft (rib graft) was harvested, shaped and positioned to improve the aesthetics of the nose. Bulky nose tip was corrected by removing the prominent lower lateral nasal septal cartilage and Weir excision done to reduce the width of the alar base. Results are immediate. This is a case of deviated nasal septum and depressed dorsum of the nose. Maxillofacial Surgeon Dr. S. M. Balaji cosmetically corrected the deviated nose by closed rhinoplasty technique. Deviated nasal septum was straightened and dorsum of the nose was augmented by costochondral autograft obtained from the rib cartilage. Successful removal and reconstruction of huge cystic lesion of lower jaw enhancing the functional stability

A 20 year old boy reported to our hospital with the complaints of swelling in right side lower half of the face. He also said that this swelling was there for a very long period with occasional numbness in the right side.

After thorough clinical, radiological and histopathological examination, Maxillofacial Surgeon Dr. S. M. Balaji diagnosed it as odontogenic keratocyst (OKC) involving right body and ramus of the mandible. He planned to remove the entire lytic lesion and reconstruction of the affected portion of the mandible in the same surgery. Costochondral graft was harvested from the ribs to reconstruct the mandible. Through intraoral approach, after raising gingivomucoperiosteal flap extending distally, the entire lesion along with the affected portion of mandible was removed in toto. Harvested rib graft was used to reconstruct the defective region of mandible. Patient is very happy to have both removal of lesion and reconstruction of the affected portion of mandible in the same surgery.

Cosmetic augmentation rhinoplasty without any scars

A 24 year old girl reported to our hospital seeking cosmetic correction of her nose. She felt the dorsal surface of her nose was flat which gave her an aged look. She wanted to correct her nose profile cosmetically without any scar.

Maxillofacial Surgeon Dr. S. M. Balaji successfully planned to correct her flattened nose by following closed augmentation rhinoplasty. Costochondral rib graft was harvested which was used to augment the dorsum of nose. To prevent scarring, the graft was inserted through intranasal approach. Results are immediate, patient is very happy to have a pristine nose without any surgical marks, which suits her facial profile.
