Cosmetic Rhinoplasty for broad nose deformity

This young man had always been dissatisfied with the shape of his nose. He had always felt it was very broad and had a hump. This made him feel depressed and socially withdrawn. His parents therefore decided to seek surgical correction of his nasal deformity.

It was explained to him that a closed rhinoplasty would be performed to correct the imperfections in his nose. There would be no visible scarring from this procedure.

A transcartilaginous incision was made and dissection was done to the lower lateral cartilages. The lateral cruces were excised to reduce the breadth of his nose. Dissection was then done to the dorsum of his nose. His nasal hump was rasped down to give his nose a straight profile.

Bilateral Weir excisions were then performed to reduce the breadth of his alar base. This resulted in a nasal structure that was in harmony with the rest of his face.

The patient and his parents were very happy with the results of the surgery. They expressed their satisfaction before final discharge from the hospital.

Cosmetic Rhinoplasty and Augmentation of Alveolar Defect with Bone Graft

This young man had been born with a unilateral cleft lip and palate defect, which had been corrected when he was an infant. He now presents with a depressed right alar base arising from a maxillary alveolar bone defect and fistula. Treatment planning was explained to him and he consented to surgery.
​
Lyophilized bone grafts were utilized to augment his alveolar bone defect and fistula. The bone graft was then utilized to augment the bridge of his nose through an intercartilaginous incision after excision of the lower part of his medial nasal cartilage.

Bilateral lateral osteotomies were finally performed resulting in esthetic correction of his nasal deformity. He expressed his delight at the result of the surgery.

A 33-year-old woman reported to our hospital with the complaints of a broad and flat nose; stating that it gave her an aged appearance. The patient was psychologically disturbed and requested a nose correction.
Under the skillful hands of Maxillofacial Surgeon Dr. S. M. Balaji, successfully nose correction was done by Augmentation Rhinoplasty technique. The costochondral graft was harvested from the ribs, shaped and used to reconstruct the nose to increase the height. 22 year old men reported to our hospital seeking to correct his deviated nose which greatly affected his facial appearance. He gave a history of road traffic accident 2 years back where he had severe injury on the nasal bones. He was very much disappointed as his nose was completely deviated to the right side giving asymmetric facial appearance.
Maxillofacial Surgeon Dr. S.M. Balaji successfully performed the cosmetic rhinoplasty. Vertically nasal septal cartilage height was trimmed by medial osteotomy to reduce the height of the nose. Deviation along with the width of the nose was reduced by lateral osteotomy technique. Thus deviated nose and size of the framework of the nose was corrected from inside the nose to avoid any scars on the face (closed rhinoplasty technique).
A balance between the nose shape and facial profile was achieved for a more pleasing appearance. The patient was very happy to have a narrower, shapely nose that enhanced his facial profile with absolutely no scarring.

Cleft rhinoplasty without scars

​25-year-old men reported to our hospital seeking specialized treatment to correct his cleft nose defect. He was previously operated for cleft lip and palate elsewhere. Due to the cleft defect his nose was collapsed on the left side and it affected his facial appearance.
Maxillofacial Surgeon Dr. S.M. Balaji successfully performed the rhinoplasty or nose correction. A costochondral graft (rib graft) was harvested and used to reconstruct the collapsed nasal framework. The surgery was approached from inside the nose so there was no scar. Secondary lip repair was also done to correct the vermillion defect. Immediately after surgery, the nose appeared sharp and definite giving a more pleasing appearance. Surgical correction of nose deformities also known as the “Nose Job” requires expertise and experience.  The surgeons estimate the type of defect - either excess or deficit of the height, deviations, depressions, slants and abnormally angled noses. The patient’s expectation of his/her nose is understood after careful examination.
Shown is a case with of a deficiency of the bridge portion of nose with bulky tip. Note the midlevel of the nasal bone, where there is a deficiency creating a huge impact on the patient’s facial profile. This gives the nose a saddle-shape. Maxillofacial Surgeon     Dr. S.M. Balaji planned a closed rhinoplasty technique wherein the surgical approach is through the nose to prevent unsightly scars.Appropriate measurement of nose was made and the amount of augmentation needed was noted. Autologous graft (rib graft) was harvested, shaped and positioned to improve the aesthetics of the nose. Bulky tip of the nose was also corrected intranasally by removing excessively curved lower lateral nasal septal cartilage. The patient was very happy with the surgery outcome and her enhanced appearance.

A 24 year old girl reported to our hospital seeking cosmetic correction of her nose. She had undergone nose correction operation elsewhere to correct her crooked nose shape and blocked nose but they had used open rhinoplasty technique which resulted in an unsightly scar. She was not at all satisfied with the results. She felt the dorsal surface of her nose was flat which gave her an aged look. She wanted to correct her nose profile cosmetically.
Maxillofacial Surgeon Dr. S. M. Balaji planned to correct her flattened nose by following closed augmentation rhinoplasty. Costochondral rib graft was harvested which was used to augment the dorsum of nose. To prevent scarring, the graft was inserted through intranasal approach. Enlarged turbinates were also trimmed. The patient came with a complaint of a hump in the middle portion of his nose with bulky tip. The excess bone and excess cartilage that was causing the hump was trimmed. Following lateral and medial osteotomy, lower lateral nasal septal cartilage was removed thus nose was made pristine. There were no visible scars as the entire cosmetic procedure was performed through the nose. Following surgery, the patient had an ideal sharp nose that best suited his face.

​A 23-year-old girl came to our hospital seeking to correct her nose and chin defect. Her chin was very retruded and the tip of her nose was very bulky. She was very self-conscious about her appearance and wanted to improve her facial features.
Maxillofacial Surgeon Dr. S.M. Balaji successfully performed a combined nose and chin correction. Advancement genioplasty was done to correct the retruded chin. The chin bone was sectioned, brought forward and fixed in a more balanced position. The bulbous nose tip was corrected with reduction rhinoplasty.  Foot plate of the medial crus and lower lateral nasal cartilages were removed giving the nose a more slender and sharp. Combined nose and chin correction for improved appearance

A 23-year-old girl came to our hospital seeking to correct her nose and chin defect. Her chin was very retruded and the tip of her nose was very bulky. She was very self-conscious about her appearance and wanted to improve her facial features.

Maxillofacial Surgeon Dr. S.M. Balaji successfully performed a combined nose and chin correction. Advancement genioplasty was done to correct the retruded chin. The chin bone was sectioned, brought forward and fixed in a more balanced position. The bulbous nose tip was corrected with reduction rhinoplasty.  Foot plate of the medial crus and lower lateral nasal cartilages were removed giving the nose a more slender and sharp. With these combined procedure the patient was very happy to have a more enhanced facial profile without any surgical scars.

Nose correction & Abbe flap procedure for lip correction in cleft patient

A 25-year-old man came to the hospital wanting to correct his lip and nose defect to improve his appearance. He was born with bilateral cleft lip and palate which was previously operated elsewhere. His upper lip was very thin and his nose appeared deformed making him very self-conscious about his appearance.

Maxillofacial Surgeon Dr. S. M. Balaji successfully performed an abbe flap procedure for lip correction and nose correction. A flap of tissue was taken from the lower lip, rotated across the mouth and used to reconstruct the upper lip with the base of the flap still attached to the lower lip to maintain blood supply. The flap was taken with hair follicles to ensure moustache growth. Once blood supply was established, the flap was divided giving the upper lip a more natural, improved form. The broad nose was reshaped to make it more proportionate and balanced. Following surgery the man was very happy to have an improved facial profile.
