Correction of broad and flat nose without any scars on the face

A 33-year-old woman reported to our hospital with the complaints of a broad and flat nose; stating that it gave her an aged appearance. The patient was psychologically disturbed and requested a nose correction.
Under the skillful hands of Maxillofacial Surgeon Dr. S. M. Balaji, successfully nose correction was done by Augmentation Rhinoplasty technique. The costochondral graft was harvested from the ribs, shaped and used to reconstruct the nose to increase the height. The entire surgery was performed within the nose (closed rhinoplasty technique) with no visible scars on the face. The result was immediate and the patient was elated to see her enhanced profile of the nose without any scars.

Surgical correction of  the upper lip and nose defect in bilateral cleft lip and palate patient using Abbe Flap technique

Successful reconstruction of the upper lip attempts to maintain the anatomic and physiological relationship of the philtrum (central portion of upper lip) and the base of the nose.

This is the case of secondary cleft lip and nose deformity in a 22 year old boy from Nepal. He is a known bilateral cleft lip and palate patient where the primary cleft lip and palate surgeries where done elsewhere in his hometown. He had defective appearance of the nose and lip due to which he lost his self confidence and he was very conscious about the appearance.

Maxillofacial Surgeon Dr. S. M. Balaji successfully corrected both the nose and lip defect together using Abbe flap technique. In the first stage a flap of tissue was taken from the lower lip, rotated across the lips and used to reconstruct the thin central portion of the upper lip, with the base of the flap still attached to the lower lip. Simultaneously in the same procedure itself, reduction rhinoplasty was done to correct the broad nose. After blood supply was established in a few days, the second stage surgery to divide the flap was done. The defect was corrected giving the upper lip a more natural, enhanced form. Results are immediate, both the nose and upper lip defect are corrected giving improved appearance and this boosted the boy’s confidence.

Successful correction of bulky nose without any scars

24 year old women reported to our hospital with the complaints of broad flattened nose which gave her an aged appearance. She felt that her nose profile doesn’t suit her face and she was psychologically disturbed due to her appearance.

Maxillofacial Surgeon Dr. S. M. Balaji successfully corrected her nose by closed reduction rhinoplasty technique. During procedure, caudal portion of lower lateral nasal septal cartilage was removed. Bridge of the nose was made narrow by lateral osteotomy technique. Results are immediate. Patient was happy to have reduced shape and size of the nose giving pristine appearance without any surgical marks on her face.

Cleft rhinoplasty without scars

​25-year-old men reported to our hospital seeking specialized treatment to correct his cleft nose defect. He was previously operated for cleft lip and palate elsewhere. Due to the cleft defect his nose was collapsed on the left side and it affected his facial appearance.
Maxillofacial Surgeon Dr. S.M. Balaji successfully performed the rhinoplasty or nose correction. A costochondral graft (rib graft) was harvested and used to reconstruct the collapsed nasal framework. The surgery was approached from inside the nose so there was no scar. Secondary lip repair was also done to correct the vermillion defect. Immediately after surgery, the nose appeared sharp and definite giving a more pleasing appearance. The patient was very happy to have his nose defect corrected without any visible surgical marks on the face.

A 24 year old girl reported to our hospital seeking cosmetic correction of her nose. She had undergone nose correction operation elsewhere to correct her crooked nose shape and blocked nose but they had used open rhinoplasty technique which resulted in an unsightly scar. She was not at all satisfied with the results. She felt the dorsal surface of her nose was flat which gave her an aged look. She wanted to correct her nose profile cosmetically.
Maxillofacial Surgeon Dr. S. M. Balaji planned to correct her flattened nose by following closed augmentation rhinoplasty. Costochondral rib graft was harvested which was used to augment the dorsum of nose. To prevent scarring, the graft was inserted through intranasal approach. Enlarged turbinates were also trimmed. ​A 23-year-old girl came to our hospital seeking to correct her nose and chin defect. Her chin was very retruded and the tip of her nose was very bulky. She was very self-conscious about her appearance and wanted to improve her facial features.
Maxillofacial Surgeon Dr. S.M. Balaji successfully performed a combined nose and chin correction. Advancement genioplasty was done to correct the retruded chin. The chin bone was sectioned, brought forward and fixed in a more balanced position. The bulbous nose tip was corrected with reduction rhinoplasty.  Foot plate of the medial crus and lower lateral nasal cartilages were removed giving the nose a more slender and sharp. A 25-year-old man came to the hospital wanting to correct his lip and nose defect to improve his appearance. He was born with bilateral cleft lip and palate which was previously operated elsewhere. His upper lip was very thin and his nose appeared deformed making him very self-conscious about his appearance.

Maxillofacial Surgeon Dr. S. M. Balaji successfully performed an abbe flap procedure for lip correction and nose correction. A flap of tissue was taken from the lower lip, rotated across the mouth and used to reconstruct the upper lip with the base of the flap still attached to the lower lip to maintain blood supply. The flap was taken with hair follicles to ensure moustache growth. Once blood supply was established, the flap was divided giving the upper lip a more natural, improved form. The broad nose was reshaped to make it more proportionate and balanced. Following surgery the man was very happy to have an improved facial profile.

Combined nose and jaw correction for enhanced appearance

A young man came to our hospital wanting to correct the shape of his broad, bulky nose that gave his face an unattractive appearance. Also his lower jaw was severely protruding making it difficult for him to bite, chew and speak well. He wanted to correct his jaw defect also.

Maxillofacial Surgeon Dr. S.M. Balaji performed a combined nose correction and jaw correction surgery. The large bulky nose was reduced and made more proportionate and sharp with reduction rhinoplasty. Using Obwegeser’s sagittal split osteotomy the protruding lower jaw was set back and into proper alignment with the upper jaw thereby correcting his bite. Nose and jaw correction was done from inside the nose and mouth respectively so there were no scars.  The man was very happy that his appearance improved greatly and that both his nose and jaw defect were corrected simultaneously.

A 22 year old woman reported to our hospital seeking to correct her deviated nose which greatly affected her facial appearance. She was very much disappointed as her nose was completely deviated to the right side giving asymmetric facial appearance.

Maxillofacial Surgeon Dr. S.M. Balaji skillfully performed the cosmetic rhinoplasty. Vertically nasal septal cartilage height was trimmed by medial osteotomy to reduce the height of the nose. Width of the nose was also reduced by lateral osteotomy technique. Caudal portion of lateral nasal septal cartilage was removed to correct the nose tip. Thus deviated nose and size of the framework of the nose was corrected from inside the nose to avoid any scars on the face (closed rhinoplasty technique).

A balance between the nose shape and facial profile was achieved for a more pleasing appearance. The patient was very happy to have a narrower, shapely nose that enhanced her facial profile with absolutely no scarring.

A 26-year-old woman came to our hospital seeking to correct the shape of her nose to enhance her appearance. Her nose was very large and bulbous. Also there was a mole on her nose which she wanted to be removed.

Maxillofacial Surgeon Dr. S.M. Balaji successfully performed the rhinoplasty to correct the nose shape. Lateral osteotomy was done and lower lateral nasal septal cartilage was removed to reduce the bulky nose and decrease the size of the nose tip. Weir excision was done to narrow the wide nostrils. A 30-year-old man from Bangladesh reported to our hospital seeking to correct his nose and lip defect to improve his appearance. He was previously undergone multiple surgeries elsewhere for his cleft defects. His nose appeared deviated to the left side, his nostrils appeared collapsed and his upper lip was short and thin that gave him an unattractive appearance.

Maxillofacial Surgeon Dr. S.M. Balaji successfully performed a combined correction of the nose & lip defect. The nasal septal deviation was corrected.  Dome binding sutures were strategically placed to narrow the lower lateral nasal cartilage, thus making the nose tip less bulbous. The nostrils were reshaped and this enhanced the appearance of the nose instantly.

The lip defect was corrected with Abbe-flap technique. A flap of tissue was taken from the lower lip, rotated across the mouth & sutured to the upper lip with the base of the flap still attached to the lower lip to maintain blood supply. The flap was taken with hair follicles to ensure moustache growth. After 3 weeks once blood supply is established, the flap will be divided, giving the lips an enhanced form, function & appearance.

Simultaneous nose & jaw correction and lip correction with Abbe flap in cleft child from Seychelles

A 13-year-old boy from Seychelles, born with bilateral cleft lip and palate was brought to our hospital by his parents for expert cleft treatment. He was operated elsewhere for primary cleft lip and palate. He had a thin upper lip, broad flattened nose and protruding lower jaw for which he sought treatment.

Maxillofacial Surgeon Dr. S.M. Balaji performed the specialized Abbe flap technique to correct the upper lip. A 30 year old man from USA reported to our hospital seeking correction of his facial appearance. He was not happy with his broad flattened nose and upper jaw prominence giving him more aged appearance. He wanted to have cosmetic correction of his nose and jaw without any surgical marks on his face.

After thorough clinical and radiological examination, Maxillofacial Surgeon Dr. S.  M. A costochondral rib graft was harvested and used to reconstruct the nasal framework. Dorsum of nose was augmented and alar base reconstruction was done through weir incisions. Her nose was flattened and collapsed on the left side and there was an unsightly scar on the lip which affected her appearance. She was previously operated elsewhere for her unilateral cleft lip and palate in her childhood.

A 35-year-old woman reported to our hospital seeking to improve the structure of her nose to improve her facial appearance. The bridge of the nose was depressed and appeared excessively concave in the profile view. Also the tip of her nose was very stout & bulky.

A costochondral cartilage graft was harvested from the rib and used to reconstruct the depressed dorsum of the nose. Excessive fat was removed from the nose tip to make it proportionate & pristine. The procedure was done inside the nose so there was no scar. Following rhinoplasty, the patient was very happy to have a sharp, defined nose making her face more attractive.

Augmentation rhinoplasty for enhanced appearance

A 22 year-old girl reported to our hospital seeking to correct the shape of her nose. Her nose was broad, flattened and appeared humped in the profile view. She wanted a more attractive nose that would enhance her appearance.

Maxillofacial Surgeon Dr. S.M. Balaji skillfully performed the augmentation rhinoplasty. A costochondral cartilage graft was harvested and used to reconstruct the deformed dorsum of the nose. The graft was placed from inside the nose so there was no scarring. The nose was made sharp, defined and pristine which greatly enhanced her facial appearance. The girl was very happy with the surgery results.

A 20-year-old man reported to our hospital seeking to correct the appearance of his nose. His nose was much flattened in the middle and he wanted to have a sharp nose to improve his appearance.

Maxillofacial surgeon Dr. S.M. Balaji expertly performed the augmentation rhinoplasty. A costochondral cartilage graft was harvested and used to reconstruct the depressed, collapsed dorsum of the nose. The surgery was done from inside the nose (closed rhinoplasty) so there was no scarring. The patient was very happy to have a sharp, defined & pristine nose immediately after surgery, which improved his appearance.
